The workshop is taught by leaders who were trained by Stanford University and all of the materials that will be used were developed by Stanford University's Chronic Disease-Self Management Program. This program is designed to help individuals who suffer from a chronic condition such as diabetes, asthma, heart disease, etc. Each participant will be provided with a Living Well with Chronic Conditions manual.
