This FREE workshop is open to the public. The workshop is taught by leaders who were trained by Stanford University and all of the materials that will be used were developed by Stanford University's Chronic Disease-Self Management Program. This program is designed to help individuals who suffer from a chronic condition such as diabetes, asthma, heart disease, etc. Each participant will be provided with a Living Well with Chronic Conditions manual.
